Below is a list of all pertinent documents for DGHI fieldwork, funding, and the Global Health Certificate.

Global Health Certificate Related Documents

GHC Advising Checklist- Advising checklist of all Global Health Certificate requirements.
Graduate Certificate Program Application- Application to pursue a certificate through the Duke Graduate School.  This is only necessary for Graduate School students and non-degree graduate students to enroll in the GHC program.

      Sample Fieldwork Projects- Here are some of the past fieldwork projects GHC students have done.
   
    Pre-fieldwork
    GHC fieldwork approval form- Is submitted to the GHC Certificate Director in order to have a fieldwork experience fulfill the GH certificate requirement.  It is advised (but not required) that students discuss the proposed fieldwork project with either the Student project coordinator, or the GHC program coordinator before submitting the approval form.
    Community Collaborator Statement of Commitment- Statement of commitment for the Global Health Certificate student’s community collaborator to complete and submit before the student’s fieldwork experience, stating that the community collaborator is willing to assist in housing and acculturation as well as mentor the student.
    Letter of Interest Instructions- Instructions on how to prepare a Letter of Interest when applying to a DGHI project (see Current DGHI Fieldwork Projects).  Not necessary if not applying for a DGHI project.


    Post-fieldwork
    Community Collaborator Post-Project Evaluation (.PDF) (.DOC)- This work statement will be completed at the end of the student’s fieldwork experience by the community collaborator stating the details of the student’s project, assessing the student’s performance and the project’s overall impact.
    Post Fieldwork Requirements- A guide to the post-fieldwork report, and other tasks required of Global Health Certificate students. 

Funding

    Grant Application and Fieldwork Planning Guide- DGHI fieldwork grant application.  It is also a useful tool to those who would like structure when planning their fieldwork experience, but are not applying for a DGHI grant.
    Funding Global Health Fieldwork Projects- A guide to how to find funding from various sources for Global Health fieldwork projects.
    Student Conference Presentation Grants- The grant application for students who are presenting research at a conference or event.
